Health Policy and Agreement

By checking and signing below, I agree that any dog that I bring to the Opportunity Barks Inc. facility, is healthy and up to date on all age-appropriate vaccinations or titers, including distemper, leptospirosis, bordetella, and rabies.  Dogs cannot come to our facilities if they have had diarrhea, excessive coughing, have been lethargic, or thrown up in the previous 24 hours.

I agree that I, as the owner, am responsible for any/all medical bills for my dog, and Opportunity Barks Inc. is not responsible for any medical treatment, vetting, transport, or fees associated with such while in their care, whether from accident, illness, injury, or environmental factors. I will pay the Vet directly for any charges on my dog’s behalf if care is needed, and if I am not available to do so, Opportunity Barks Inc. has permission to charge my card on file for reimbursement for any Vet care needed while in the care of Opportunity Barks Inc.

Liability Waiver

Opportunity Barks Inc. (OpBarks) will endeavor to create as safe an environment as possible while offering sound, safe, and responsible care and recommendations to their clients. However, I recognize that OpBarks is not responsible for any and all unintentional errors, omissions, environmental interactions, or incorrect assertions, and I understand that training alone may not cure behaviors that I am seeking help for.

I hereby release and agree to save and hold harmless, Opportunity Barks, Leigh Siegfried, any affiliates, agents, or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused or inflicted by my dog(s).

I acknowledge and agree that Opportunity Barks is a separate and independent business from the owners of the property where its facilities are located and the owners of the property are not responsible for any claim I may have arising out of or in connection with any services provided by Opportunity Barks.

I, the undersigned owner/handler (client), do hereby agree that Leigh Siegfried, Opportunity Barks Inc, and all Opportunity Barks employees and agents will not be liable for any damage or loss, such as; damage from disease, death, running away, theft, fire, injury to or from persons, other dogs or property, or by any other causes resulting from the failure of the dog to respond to any cues or behaviors taught by Opportunity Barks Inc or resulting from counseling and advice supplied to the owners/handlers (client) of the dog.

Photo and Video Waiver

I hereby assign and grant to Opportunity Barks (OpBarks) the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s) by Opportunity Barks, and I hereby release Opbarks from any and all liability resulting from such use, reproduction, distribution, and publication.

I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of Opbarks and I specifically waive any right to any compensation I may have for any of the foregoing.

Appointments cannot be canceled or rescheduled within 72 hours of the confirmed date/time. Any rescheduling of appointments within this 72-hour timeframe will result in a $50 transfer fee for Evaluations, Private Lessons, Puppy Socials, and Puppy Classes.

Once confirmed, Drop-Off Day Training program reservations are locked in for the dates and times selected. If Day Training programs are rescheduled within 7 days of the program start date, we maintain the right to keep 50% of the program fee as a non-refundable deposit.
